FUT hair transplantation method

Follicular Unit Extraction (FUE) method is the most modern hair transplantation technique which is applied in the world and Turkey. After hair follicles are extracted, the results will be natural. In this process, living hair follicles are taken and they are transferred one by one into bald part of the head.The difference between FUT and FUE methods is, in FUE method follicular units (grefts) are extracted directly from follicle level of the skin. This process is known as “non-invasive” method. Scalpel is not used in the donor area while FUE method is being performed, suture is not carried out, any scars will not appear after the operation, any complications won’t occur and daily life of patient will not be delayed. While extraction process is different in FUE and FUT methods and transplantation process is exactly the same in two methods.
When Dr. Rassman explained scientific consequences of FUE method which is called as F.O.X Procedure by him, our Surgery Center made researches about this extraction technique. First, in June 2004, we performed a FUE operation. The method which is being used now is an enhanced type of the technique which is created by Dr. Ray Woods and developed by Dr. William Rassman.
Baldness is a problem which has physical and psychological effects on person. From the age 18, every one of nine males looses its hair completely, and the other males are partly affected, they loose their hair partly.
In transplantation process which is performed by our team, between 1300 and 1800 follicular units (GREFT) are transferred.
FUE is a kind of revolution which puts modern aesthetics surgery together with art as micro surgery. It opens new roads for people who are without hair and helps them be free from this problem. And it leaves the other hair transplantation methods with its natural consequences.
Outcomes of FUE and FUT operations are at the same level in terms of quality. Only difference between the two operations is extraction method of follicles from the donor part. Transplantation process is like in the FUT method. There is no difference in FUT and FUE methods which are applied in our clinic in terms of quality of outcomes. This method has been applied to our patients with world standards and our experience since June 2004.
Situations where Follicular Unit Extraction needs to be selected:
After the operation, if whole head will be shaved sometimes, FUE treatment is appropriate. When length of the hair is shorter than 0.5 cm, the thin mark on the donor part might be seen.If small amount of follicles is enough for the part which is needed to be hairy, FUE method is preferred in our clinic. For open areas on temples, and treatment of burns and marks, FUE is an appropriate method.
When there are no enough donor areas. FUE enlarges the area of donor source. Especially, possibility of hair extraction from other parts of the body increases number of follicles. Due to some patients who are young and who have high skin elasticity have tendency towards large scar improvement, FUE is preferred.
FUE is partially effective method for transplantation of beard, mustache, and eyebrows.
Stages of FUE Process:
First, nucha hair is shaved. A larger area needs to be prepared considering the other method.
After the area where follicles will be extracted is given anesthesia, follicles are extracted one by one with quite thin needles which have 0.9 – 1 mm caliber.
Grefts which are obtained with this method are composed by follicular units. And they do not need to have any operations.
Until reaching the desired number of grefts, operations above can be performed once in two days again.
Operation is performed by a doctor from start to the end. There is no need for a crowded team for the operation.
With FOX test which will be made before the FUE method, we can determine whether patients are suitable for hair transplantation with this method, or not.
This method is preferred by people who are reluctant to surgical process; however this method has some disadvantages, too.
DISADVANTAGES OF FUE METHOD
The donor area needs to be shaved in order to make length of hair as 1 or 2 millimeters. This can create a problem when we use nucha as the donor area. Because after the operation, the patient has to have very short hair on his nucha. This problem can be removed if patient has his hairs which exist on right and left sides cut very shortly.
The operation lasts longer considering the FUT method. Number of greft which can be taken in one day is between 1300 and 1800.
In order to extract and transplant this amount of grefts, we need 7 – 9 hours. For example; when we make operation on a patient who needs 2000 grefts, we need 9 or 10 hours. For this reason, we prefer patients who need less greft number.
Because of the operation is completed in 2 days and it is more tiring considering the other method, it is more expensive than the FUT technique.

What is Follicular Unit?
In this process, hair follicles are extracted in the form of bands from nucha where hairs are resistant to fall and they are divided into follicular units which have 2 or 3 hair follicles without damaging integrity of the hair. Because number of these units is more than the number of hair follicles which is composed by mini-micro greft technique and because they are not worn; they enable more hair follicles to be transplanted.All of the hairs on the head skin grow in the form of little bunches called as “follicular units”. The follicular unit on an adult person’s head skin, is composed of hair follicles which have complete thickness (1-4 terminals). In some areas of the head which are affected by hair loosing genetically, small scale and short hairs which are called as “miniaturized hairs” start to replace healthy terminal hairs. Follicular unit looks as if it grows in the form of small hair groups on the surface of head skin.
How can a Follicular Unit be obtained?
Hair density of follicular units per cm2 is counted and measured before in order to obtain the necessary amount for the area where hair transplantation will be applied.
With necessary estimations, the most suitable band in terms of width and length is obtained in order to have the desired hair density.
Grefts are extracted from arrear of head skin with surgical operation. The tissue is extracted in one piece so any damages cannot affect it. This technique is quite important feature of Follicular Unit Transplantation. Because not only the follicular units stay undamaged; but also every single hair follicles are protected from getting damaged. The greatest difference between this technique and mini-micro grefting technique is, in this technique, blades are not used in order to extract follicular units. So they stay as single parts; they are not divided into pieces.
Healing the donor area and scar which appears after the operation
The area where operation is performed is enclosed by suturing with special surgical yarns and sutures are taken after 15 days. The scar will not appear as long as you have not your hair cut too short. Because this method does not give any harm to natural integrity of head and structure of hair; results after the operation will be quite natural.
Where and When Follicular Unit Extraction is applicable?
• It is applicable for patients who do not think to have not their hair cut as 0 degree or shorter than 0.5 cm.
• It is applicable for patients whose time is limited and who want hair transplantation process to be quick.
Advantages of FUT Technique:
• In this process, the maximum amount of hair follicle is transplanted in the shortest time.
• If there is need for more hair follicles, this process can be combined with other techniques in the same day (FUT + FUE)
• Natural structure of hair groups is protected and so maximum growth rate is achieved.
• In this process, channels in the donor area will be smaller. And this leads the transplantation process to be natural and hairs to be more dense. Because of channels are small, they do not damage blood circulation, hair follicles can be nourished by the blood. So hairs grow faster.
• In the FUT method which is performed under stereoscopic microscopes, hair is extracted by shaving without any damages. So grefts which include only hair and whose tissue is not exposed to any harms can be obtained. These grefts can be directed in the desired way because they pretend the natural structure of hairs.
• In the FUT method, channels are opened in the form of very thin scratches called as “slit”. By using slit technique, the operation could be performed without giving any harm to hair follicles. So, it is quite easy to increase the number of hairs.
Disadvantages of FUT Method:• Although it is a painless method, because surgical operations are performed, the open sore needs to be sutured.
• If patient has his hair cut too short, a thin scar can be appeared on nucha.

• Quit taking aspirin or other anti-inflammatory drugs (ibuprofen, ecopirin, alka seltzer, etc.) 2 weeks before the operation.
• Quit drinking alcohol 3 days before the operation.
• Tell your doctor if you are taking pills because of chronic or acute syndromes (diabetes, heart disease, tension, tuberculosis, etc.).
• Quit taking antidepressants, some other drugs which prevent coagulation (heparin, coumadin, etc.), beta blockers, and mao-inhibitors (aurorix, etc.) 2 weeks before the operation. During this time, if you take one of these drugs please tell your doctor.
• Do not apply any local medicines on to head skin and hair before the operation.
• Before coming to operation, wash your hair and do not use hair spray or hair gel.
• Have breakfast before the operation. If your operation will start afternoon, have light lunch.
• Please bring a hat with you in order to wear after the operation.
• Quit smoking 24 hours before the operation; if you cannot do that please reduce the number of cigarettes you smoke. Remember that cigarette will prolong the duration of recovery, and will increase the risk for infection and scars.
• Do not drink coffee or other drinks which include caffeine on the operation day. These kinds of drugs increase your bleeding and your sensibility to drugs which need to be taken after operation.
• Stop taking multivitamin preparates which consist vitamin B and E one week before the operation; because they may increase your bleeding.
• Do not have your hairs which are on arrear part of your head cut, because they will be used for closing the suture area after the operation.
• After the operation, do not drive a car for 12 hours; if you come to operation with your car, please bring some one with you to drive the car after the operation.
• While you are coming to operation, please wear comfortable cloth like shirt or cardigan.
After your operation:
Your feelings after operation are depended on type and comprehension of the process. If you feel any pain or discomfort, you can take them under control by using pain killers.
If you wear bandage, generally it is opened the next day. 2 or 3 days after the operation, you will start to wash your hair and continue to wash it as you were told until your sutures will be removed. Sutures will be removed 15 days after the operation.
Heavy activities might lead grefts and surgeon areas to bleed because they increase blood flowing to the head. For this reason, you need to avoid heavy activities and making sports for 3 weeks. Sometimes doctors recommend patients not to have sexual relation for 10 days after the operation.
1. Take the pills which are given by the doctor in the way that he / she told to you.
2. In three days after the operation you should avoid to make heavy activities, feel over excited, be constipation, make sports, have sexual relations. When you are going out, wear your hat.
3. You can start your daily activities 1 day after the operation.
4. You can start to make light exercises two weeks after the operation.
5. You should not to smoke until your sutures are removed; that is, until the 15th day. If you cannot do that, please do not smoke during the first three days.
6. You should not be exposed to the sun without protection during the first three months. When you are under the sun, please wear a hat.
7. After the operation, you will experience a feel loss for a while like in other operations. Reason of this is little nerves which are cut while skin band is being taken from this part. After one or two months, when these nerves are renewed, your feel loss will disappear.
8. Do not drink alcohol in the period that you take drugs.
9. After the operation, a swelling might appear on your forehead. Because during the operation some drugs are applied under the skin and these drugs move in the downward direction due to gravity. So they create swelling on your forehead. In this case, you can apply ice compress and you need to use drugs regularly. Even if you do not make any intervention, these swellings will disappear in two days. If you wear contact lenses, 2 or 6 days after the operation, please do not wear them.
10. Please protect your head from any traumas during 2 weeks after the operation.
11. After the operation, small amount of bleedings may occur on your nucha. Patients frequently encounter with this case, so do not panic. 1 day after the operation, if bleeding continues, please call your doctor.
12. You can wash your hair 2 or 3 days after the operation as described below.
13. After the operation, until the 15th day, when you wash your body do not touch water to your hair, wash parts of your body which are under your neck.
14. 2 or 3 days after the operation, you will wash your hair once or twice in a day like that: put the lotion in your hair and apply it to your head where the transplantation process was performed by compressing it. Wait lotion dry for 1 or 1.5 hours. And then, clean your head with warm or cold water. Then, put the shampoo in your hand, and apply it to your head by compressing it. Do not wait. Clean your head with warm or cold water. Dry your head with a soft towel without scrapping. Please continue to wash your head in this way until your sutures will be removed; that is, until the fifteenth day.
Washing hairs after the operation:
You are going to wash your hair first, 2 or 3 days after the operation. You will wash your hair with a special lotion and shampoo, everyday, once or twice in a day until the fifteenth day when your sutures will be removed. You will wash your hair as described below:
Put the lotion in your hair and then apply it on to your hair by compressing, not by scrapping. Lotion stays for 1 hour on the hair in order scabs and clots to be soft. Then the transplantation area will be cleaned with warm or cold water hair not by scrapping, by compressing. Then the special shampoo is applied by compressing, and hair will be cleaned with warm or cold water not by making the shampoo wait on the hair.
After you dry your hair with a soft towel by compressing, you or someone else will apply the special lotion which is given by our clinic to sutured area which is located between the two ears with a cotton or gauze dressing. Until the fifteenth day after the operation, you will wash your hair once or twice in a day in this way. After the fifteenth day, you can wash your hair in the old way. In that period, you may wash parts of your body which are under your neck.
